  Case #13 The baby looks fine!

An 11 month old female presents after an accidental submersion in a bathtub. Mom reports she initially found the infant without signs of life, so she started CPR and called 911. On EMS arrival, the baby was breathing, had a pulse, but still limp. Bag-valve-mask assistance for breathing was initiated with 100% oxygen. By the time the baby arrived in the ED, she was crying, had a pulse of 120, R = 45, BP = 120/50, and T=98F. SAT=95% with oxygen.

PE: Crying, Good eye contact, consolable by mom
PERRL
No evidence of head trauma
Fontanel normal
Heart RRR no M
Lungs: Wheezing and scattered rales
Abd: normal
Ext: pulses present, cap refill normal
Neurologic: Good eye contact, consolable, moving all 4 extremities, normal tone

1. What is the difference between drowning and near drowning? What will be your admission diagnosis?

The patient is placed on oxygen, an IV is established, the patient is placed on telemetry and sat monitor, the VS are repeated (unchanged) and blood is drawn with the IV stick. You order CBC, Lytes, BUN CR, and Glucose.

2. What other test do you need to order, and what other intervention do you need to do?

3. The nurse wants to know if you want an ABG or if you can just use blood from the venous stick for IV access and get a Venous Blood gas.
