  Case #19. Long Way Over the Hill

Chief complaint: Seizure.

A 13 y/o female presents after a generalized tonic clonic seizure at home. The seizure occurred while sitting on a couch, there was no head injury, and it lasted about 2 minutes according to the family.

PMH: Negative. Full term child, no hospitalizations, and no surgery.
FH: Sister had seizures.
SH: Negative.
ROS: Previously in good health, no fevers, chills, nausea, vomiting, diarrhea, head injury, mental status changes, headache, sore throat, rash, vision changes, or weakness.

PE: WDWN female, no longer post ictal,
HEENT normal, neck supple
Heart RRR
Lungs Clear and normal
Abd Neg
Ext normal
Neuro:
Motor: 5/5 throughout
Sensory: no deficit
Reflex Normal
Cranial nerves: no deficit
Cerebellar: normal finger to nose test, normal tandem gait
Mental status: normal

1. What neuro-imaging work up is necessary?

2. Can playing video games bring on seizures?

3. Do you need an EKG?
