The smiling crybaby
A 7-week-old infant is brought in for
persistent crying. This is the second child
for this mom, and her first baby did not cry
as much. Her doctor told her it is just
colic, but she is still concerned as she
feels something may be wrong. She wants to
know what you think. She always checks for
fever, and there is never a temperature. It
has been getting more frequent, so that now
it is happening almost every night. The baby
seems to be angry or in pain, and nothing
stops the crying until she eventually cries
herself to sleep. There is no vomiting (the
baby does spit up normally), and the baby is
gaining weight normally. Mom Breast-feeds
her baby. On physical exam, the vital signs
are all normal, the baby is smiling, and the
naked exam is completely normal. In fact,
the mom notes that the baby seems normal now
but is much different when she is crying at
home.
1. What is the rule of 3’s for diagnosing
colic?
2. Changes in the infant formula (to an
elemental formula) been suggested in case
the crying is related to allergies. What can
you suggest to a breast-feeding mom?
3. How much crying in an infant is normal?
