Return Visit Headache!
A 34 y/o female presents 2 days after a
previous ED visit for a headache. When she
left the ED, her headache was gone (they
told her after an extensive work-up,
including CAT scan and Lumbar puncture, that
she had migraines). This headache is
different from the headache she had when she
first came to the ED in that whenever she
stands up, the pain is unbearable. Her vital
signs are all normal. Her neurologic exam is
nonfocal, and the puncture site appears
normal.
1. What is the incidence of post Lumbar
puncture headache? What can be done to
minimize the likelihood of a post-lumbar
puncture headache? Who is most likely to
suffer a post-lumbar puncture headache?
2. What pharmacologic treatments have some
proven benefit, and how do you give it?
3. If you do nothing, how long with the
headache usually last?
